If you have an ACC claim or a hospital referral, we can coordinate directly with ACC or Health New Zealand (Te Whatu Ora) to arrange support.
We aim to respond quickly and make the process as simple and stress free as possible.

Rapha Health is a nurse led, locally owned provider. We take a hands on, clinical approach to home and community care not just task-based support.
Because we are a smaller provider, we’re able to offer more personalised care, consistent staff, and direct communication. Clients are not just a number in a large system.

Yes. We also offer private pay services for individuals who do not qualify for ACC or Health New Zealand funding.
If you're unsure about your eligibility, we’re happy to discuss your situation and help you understand your options. We can provide nursing care, personal support, and tailored home care plans on a private basis.

Yes. Rapha Health provides services across the Greater Wellington region, including some rural and semi rural areas.
If you're unsure whether we cover your location, please contact us. We’ll confirm availability and discuss what support options are possible in your area.
Where appropriate, some assessments and reviews can also be completed via telehealth, while ensuring in person care is delivered safely and consistently.

Rapha Health Care is committed to providing high-quality support that helps you remain independent in your home and community.
If we have not met your expectations, we encourage you to let us know. Feedback, concerns, or complaints can be submitted using this form and will be sent directly to our Quality Team. The team will work with the local service to review your concern and respond using the contact details you provide. We aim to respond within five working days of receiving your feedback.

Accessing Government-Funded Support
Access to government-funded services for older people, people with disabilities, and people with mental health needs is managed through Needs Assessment and Service Coordination (NASC) services. NASC agencies assess your needs, determine eligibility for funded support, and refer you to a provider that can meet those needs. Different NASC agencies operate across different regions of New Zealand.

Injury-Related Support and Nursing
If you have had an injury and require home support or nursing, you will need to be assessed by ACC. This usually begins with a visit to your doctor, nurse, or another health professional, or by attending an accident and emergency clinic.
Rapha Health Care also supports clients who choose to pay privately. Please contact us to discuss available options.
Mental Health Support
Government-funded mental health services are provided through Community Mental Health Services. Rapha Health Care can also support privately paying clients where appropriate.

Injury-related support and nursing
If you have had an accident and need home support or nursing you will need to be assessed by ACC. The first thing you need to do is see a doctor, nurse or health care professional, or go to an accident and emergency clinic.

Yes, in some situations we can support with childcare as part of a wider care plan. This can include supervision and age-appropriate support so a parent or caregiver can rest, recover, or attend appointments.
If childcare is needed because of an injury or health situation, support may be available through a funding plan (for example, injury-related or disability-related funding). Availability depends on your needs and safety requirements and must be approved by the relevant funder or included in your confirmed support plan. We can guide you through the next steps and help you understand what’s required.

Ways to Access Our Services

Needs Assessment and Service Coordination (NASC)
If you, or someone close to you, needs support to continue living safely at home, a NASC assessment is usually the first step to accessing funded services. Rapha Health Care works with NASC agencies across New Zealand to provide home and community-based support.
What is NASC?
NASC (Needs Assessment and Service Coordination) is a government-funded service that assesses a person’s support needs and helps arrange appropriate services.
A NASC assessor may meet with you and your whānau to understand your daily needs, health requirements, and living situation. Following this assessment, you may be approved for supports such as:
Personal care
Household support and meal preparation
Respite support
Long-term condition support
Rehabilitation or specialist services

After a NASC assessment, you can choose your care provider. Rapha Health Care works closely with NASC teams to deliver practical, reliable care that supports independence and wellbeing.
